Thank you for considering Lombard Veterinary Hospital Rehabilitation Services for your pet’s physical therapy and recovery. We are dedicated to improving mobility, health, and quality of life through our comprehensive rehabilitation programs. Our team, led by Doctors of Physical Therapy with extensive training in animal rehabilitation, are here to guide you and your pet every step of the way.

Steps to Becoming an LVH Rehabilitation Patient:

  1. Submit a New Client Application Form
  2. Email Our Rehabilitation Department 
    • Send us an email at [email protected] indicating the reason for pursuing rehabilitation services. We will respond in a timely manner. 
    • Share our External Referral Form with your veterinary hospital to fill out. (We also will send out this form to establish care).
  3. Ensure Previous Medical History and Imaging are Sent
    • Have your primary veterinary clinic send your pet’s medical records and diagnostic imaging to [email protected] so we have a better understanding of your pet’s medical history.
  4. Schedule an Initial Consultation Appointment
    • Once we receive all required information, including:
      • Completed External Referral Form from your primary veterinarian.
      • Updated vaccine records.
    • Our team will contact you to schedule your pet’s consultation.
    • We are most responsive by email however we can schedule time to talk by phone between patient care if requested. 
  5. Begin Treatment
    • Start your pet’s journey toward improved health, mobility, and quality of life with our dedicated rehabilitation team.
